1. CASE TIP: GETTING INFORMATION FROM YOUR EMPLOYER SPONSORED HEALTH PLAN
Families with a child or a parent who are denied coverage for benefits by their insurance plans have a legal right to know the specific reason for that denial. Without this information, challenging a denial is nearly impossible.
Therefore for most group employer plans you may request from your insurer a copy of your complete plan documents (the policy), the specific reason for the denial and the section of the policy they're relying on to make the denial. You are also entitled to other documents or information outside of the policy that the insurer or plan used to make the denial. If the insurer does not provide you with this information within 30 days of your written request, the insurer may be subject to fines from the Federal Department of labor of $110 per day.
Keep copies of your dated written requests for the information. After 30 days, send a letter addressed to Department of Labor, Employee Benefits Security Administration, 200 West Adams St., Ste. 1600, Chicago, IL 60606, explaining your efforts to get the information. With this letter, include the copies of your prior requests to the insurer. Send it to the Customer Service Section.
2. A GATHERING OF YOUTH AT THE CIRCLES OF LIFE CONFERENCE
